Non-resident aliens (International visitors)
People with visas - F1 (student), J1 (student/researcher), H1 (non-immigrant worker), send the following:- NRA Questionnaire for Non-employee International Visitors form
- Copy of passport
- Copy of INS form I-94 ("Arrival-Departure Record")
- Visa documentation with approval from visa sponsor
- Original receipts
- Complete name
- Home address
- Mailing address, if different from above
Persons on B class visas (B1 or B2 for visitors, WT or WB for visa waivers) will also need to complete the Certificate of Academic Activity for Short Term International Visitors form.
Per INS instruction, The ORIGINAL forms are required.
